It’s important to respond when the N.C. Division of Employment Security (DES) requests information from you about employees who have filed an unemployment claim. This information can affect unemployment benefits and possible charges to your employer account.
Request for Separation Information
When a worker files an unemployment claim and lists you as their last employer, you will get a Request for Separation Information (NC CLM 500AB). You have 10 days from the date the claim was filed to respond.
There are two ways to respond online:
- Sign in to NCSUITS to provide separation information.
- Below the My Alerts banner on the right side, find Claim Services.
- Click on the green words Click Here.
- Click on Respond to Request for Separation Information to get started.
- If you participate in SIDES (or State Information Data Exchange System), provide information through the SIDES application.
You can also mail your completed form to:
- Division of Employment Security
P.O. Box 25903
Raleigh, NC 27611-5903
Please respond as quickly as possible so that:
- Benefit payments are not delayed.
- Your account is correctly charged.
What information do I need to provide?
The Request for Separation Information will ask you to confirm details like:
- Reason for separation (Why the person is no longer working for you).
- Dates of employment.
- Any money paid after separation.
Reporting Separation Pay
When you respond to a Request for Separation Information, you must report any pay due to the employee at the time of separation. These payments may affect a person’s unemployment benefits.
Types of separation pay include:
- Regular wages for the last week worked.
- Wages in lieu of notice – Pay in place of employees working through their termination notice period.
- Paid Time Off (PTO) – Includes sick and vacation time. If you have a written vacation policy, include this with your response.
- Holiday Pay – Reported for the week earned regardless of when paid or received.
- Severance Pay – Report the dollar amount and number of weeks covered.
- Bonus (Completion/Staying/Retention) – Reported as earnings during week received.
- Pension – Lump sum or monthly.
